Avera is currently seeking Registered Nurses (RN) Case Manager II looking to deliver the best possible nursing care in Avera’s patient-focused settings.

Job Summary

  • The case manager facilitates the plan for an orderly transition of patients from the acute area setting to the next appropriate level of care. Works closely with physicians and payors to assure proper use of fiscal resources. Serves as liaison with physicians and staff to assure on going continuity.

Education and/or Experience

  • Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.
  • Bachelor's degree in nursing preferred
  • Four years related experience and/or training; or equivalent combination of education and experience preferred.

Licensure/Certification/Registration

  • Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure is required.
  • Professional certification preferred.
  • Membership in a professional organization preferred.

Work Schedule

  • Varied Shifts; 80hrs/2wks

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.  

  • $32.00 - $46.50 per hour
